One-Hour Phone Call
During this topical call, participants will:
- Provide an overview of the needs assessment process for State Title V CYSHCN Programs
- Identify ways in which engagement in the 5-year needs assessment builds transition skills of youth
- Use data to identify youth transition priorities for State Title V CYSHCN Programs
- Discuss issues of concern to Title V CYSHCN Directors and needs assessment coordinators
- Describe tools and other resources available for use in planning and conducting the needs assessment
