Information remains limited, but recent studies indicate that people with disabilities experience both health disparities and specific problems in gaining access to appropriate health care, including health promotion and disease prevention programs and services. This paper documents the issues and suggests policies for solutions.

Bright Futures www.brightfutures.aap.org is a guide that includes health and independence building screening, assessment, and teaching guides. Bright Futures has little specific transition information but gives excellent developmental anticipatory guidance for health care providers and families. In FY2008 and FY2009, some states have had Bright Futures projects (see http://brightfutures.aap.org/integrate_state_program.html; GA, LA, ME, SC, VA, WA, AZ, FL, MA, NY, OK, WI) and are promoting use of Bright Futures screening and teaching to their states’ physicians especially through Medicaid and EPSDT.

March 2010: A new online community where young adults living with mental health concerns can provide mutual support in navigating unique challenges and opportunities during the critical transition years from ages 18 to 25. Developed by young adults, StrengthofUs.org is a user-driven social networking community where members can connect with peers, share personal stories, creativity and helpful resources by writing and responding to blog entries, engaging in discussion groups and sharing videos, photos and other news.
