Learning Objectives

  • Provide a brief overview of federal legislation regarding the health and education of public school children & youth with special health care needs and disabilities;
  • Describe the process of integrating health related needs into 504 Plans & Individualized Education Programs;
  • Describe how the IEP/504 plan can prepare YSHCN and support their transition from pediatric to adult health care;
  • Discuss how Title V CYSHCN Programs can assist families and YSHCN and their schools to support the integration of health needs and competencies supporting transition from pediatric to adult health care into the IEP and 504 Plan.
