Welcome to HRTW-U!

HRTW University (HRTW-U) is designed for sharing innovative, cutting edge, and promising practices in interagency collaboration, medical home, transition/youth, and family partnerships.

We use various interactive platforms, such as email news, topical calls, podcasts, and courses on-line. In this age where information is just a click away, we want to make sure that there is a place to be able to share ideas, begin a dialogue and get some feedback from those in the know, doing the work and living it! We welcome you to these HRTW learning communities!
