Youth/ Family Education Tools: handout or wall poster AHTP - Washington Transition Timeline (multi-lingual)
The Transition Timeline for Children and Adolescents with Special Health Care Needs (forms for developmental disabilities/delays and for chronic illnesses/physical disabilities) gives ideas to help children and youth to achieve independence in their own health care and in other areas of life as they grow. The timeline is available from the Adolescent Health Transition Project as an 11" x 17" chart and can be downloaded by following the instructions provided on their web page. Timeline is available in English, Spanish, Russian, Chinese, and Vietnamese.
